Claims

Providers must file claims for STAR+PLUS within 95 days of the date of service.

There are three ways to file a claim with Bravo Health:

  1. File claims electronically through a clearinghouse.

  2. Mail paper claims to Bravo Health.

  3. File individual claims through Bravo Health's Provider Portal.

Additional Information

Bravo Health processes Clean Claims within 30 days of receipt.

Long-Term Services & Support Providers may reference LTSS Providers for claims filing guidance.

Providers should not collect payment from or bill Bravo Health Members for covered services.
