Private Fee-For-Service (PFFS) Information

Whether you are a new physician willing to participate with Bravo Health or an existing physician currently rendering services to Bravo Health Medicare Advantage members, you can immediately become a PFFS deemed provider by acknowledging the Terms and Conditions of Payment before rendering services to any Bravo Health PFFS member. It's that simple.

Bravo Liberty I (PFFS), Bravo Liberty II Rx (PFFS), and Bravo Liberty Silver (PFFS) (Bravo Liberty plans) are considered non-network, non-managed care products that pay providers on a fee-for-service basis. Bravo Health PFFS members have the freedom to choose any physician, hospital, or health care professional that accepts Medicare and Bravo Health's PFFS terms and conditions of payment.

Here's how it works:

As a provider, you have the right to choose to accept Bravo Liberty plan enrollees each time your service is needed. No contracts are necessary.

Any licensed provider who is eligible to receive payment under Original Medicare and agrees to provide covered services to our Bravo Health Liberty plan members will be automatically considered a "deemed" provider for Bravo Liberty plan products.
